New Patient Registration
All patients living within the Practice boundary are welcome to join our list.
You will be asked to complete a registration form and a medical questionnaire.
A registration medical will be offered to all new patients and this appointment may be made at the time of registration. You do not have to have a medical - this is entirely optional but can be a good idea if you are on any medication or have ongoing medical conditions.
You will be registered with the Practice and not with a specific doctor.
You may at any time express a preference for a doctor either for all your medical needs or on a case-by-case basis.
We will do our best to respect your choice.
However, not all the doctors in the practice provide all the services and specific doctors may not be immediately available.
In order to minimise the use of paper at this current time, if you would like to complete the documents at home, and email the forms to us, please follow the links below (Note - please use Internet Explorer as the browser for this) and email to the Practice at tay.comrie@nhs.scot (Please ensure you complete the registration form and patient questionnaire, thank you)
